Estimation of fractional flow reserve using magnetic resonance imaging and computational fluid dynamics

 

Coronary artery disease (CAD) is one of the leading causes of death in the world, responsible for 1 in 7 deaths in the United States and half of all deaths attributed to cardiovascular disease. The current gold standard for diagnosing CAD is fractional flow reserve (FFR), which approximates the percent flow reduction in the stenotic artery through direct pressure measurement, requiring invasive and expensive catheterization. In this work, a novel diagnostic tool will be developed combining magnetic resonance imaging (MRI) with computational fluid dynamics (CFD) to provide a non-invasive alternative to FFR. Different MRI protocols will be investigated in coronary phantom models and healthy volunteers to determine an imaging protocol that can acquire the anatomic and flow boundary conditions necessary for coronary computational modeling. These image data will then be incorporated into a CFD model to allow for the pressure map throughout the volume of interest to be solved numerically, thereby providing the non-invasive FFR approximation, and this protocol will then be tested on coronary phantom models, healthy volunteers, and patients with CAD.
